Our practice respects your right to privacy. It is important that you understand the purpose for which we collect details about your family and child’s health information and our policies on disclosure of this information.
The policy of our practice is to follow these procedures:
The information collected will be used for the purposes of providing treatment to your child. Personal information such as your child’s name, address and your health insurance details will be used for the purposes of addressing information pertaining to your child’s treatment and processing payments.
We may disclose your health information to other healthcare professionals, or require information from them if in our judgement it is necessary in the context of your child’s treatment. In that event, disclosure of your personal details will limited to information critical for treatment.
We may use parts of your health information for research purposes, study groups or at seminars. The guiding principle for such dissemination would be to add to the body of scientific literature in Paediatric Dentistry. Should this occur, the details of your child’s identity will not be disclosed without your consent.
Your medical history, treatment records, x-rays, photographs and any other material relevant to your treatment will be securely kept at this surgery. You may request to inspect or ask for copies of our records of your treatment with at least 7 days notice. We would endeavor to provide the requested information as soon within this period as possible. Please note, statutory fees will apply in relation to the types of access you seek. If you request an explanation of our records or a written summary, our usual fees for these services will apply.
If any of the information we have about you is inaccurate, please ask our reception staff to amend your details.
You can be assured that your health information record will be treated with utmost confidentiality. Disclosure will not be made to any person not involved in the overall management of your child without your prior consent.
